False Positive Thallium due to LBBB
(Left Bundle Branch Block)

The patient is a 56 year old male who was referred for an exercise stress thallium due
to LBBB (left bundle branch block).
He exercised for 7 minutes and had no chest pain. His peak heart rate was 157 BPM
( > 85% of his predicted maximal HR). There were EKG changes with exercise.
The thallium scan above shows a mild septal defect with significant
redistribution in the rest scans, consistent with ischemia.
Since LBBB can cause a false positive scan with exercise, the patient underwent
a persantine stress test (a pharmacologic stress test). The chance of a false
positive scan with persantine in a patient with LBBB is much less.
The thallium scan above shows no septal defect suggesting that the original septal
defect with exercise is a false positive scan due to the LBBB.
